Ontario Caregiver Charged After Fatal Medication Error in Long-Term Facility

A long-term care worker in Ontario has been charged following the death of a 93-year-old resident who was reportedly given incorrect medications. The incident occurred at a facility in Iroquois Falls, where the resident experienced medical distress on May 17, 2024, and later died in hospital on May 24.

Ontario Provincial Police arrested 62-year-old Michelle Bigelow of Matheson, charging her with criminal negligence causing bodily harm. The investigation was led by the South Porcupine OPP Crime Unit under the direction of the OPP Criminal Investigation Branch.

Authorities are continuing to investigate the circumstances surrounding the incident and have urged anyone with relevant information to contact the South Porcupine OPP.

This case has reignited concerns about safety protocols and oversight in long-term care facilities across the province.
